Student Working Papers

Students applying for Working Papers must bring the following documents to the Yonkers Board of Education Medical Department at One Larkin Center:

• Application form signed by parent
• Current physical examination performed within one year
• Social Security Card
• Proof of date of birth

A student should contact the school nurse for necessary materials. Proof of age and an application signed by a parent/legal guardian are required. A school physician will perform the required physical examination if no proof of physical exam is submitted. Immunization status must be in compliance with New York State Law.
