Required Student Immunizations
Students may not attend school without acceptable written proof of updated immunizations according to NYS Public Health law 2164.
The following immunizations are required for school attendance:
- Diphtheria, Tetanus toxoid with Pertussis vaccine (DTP, DTaP, Tdap, Td) 4 or more doses for PreK-5th grade. At least 3 doses Grades 6-12.
- Tdap Vaccine is mandated for students 11 years or older in grades 6-12
- Trivalent Polio vaccine (IPV, OPV), 3 or more doses for PreK and above
- Measles, Mumps, Rubella, (MMR) 1 dose by PreK, a second dose must be administered to students in grades Kindergarten-12.
- Haemophilus Influenza Type B vaccine (Hib), 1-4 doses prior to entering PreK
- Pneumococcal vaccine (PCV), 1-4 doses prior to entering PreK
- Hepatitis B, 3 doses, mandated for all students PreK and above
- Varicella Vaccine 1 dose for PreK, a second dose must be administered to students in grades Kindergarten - Grade 12.
- Meningococcal vaccine (MenACWY) 1 dose mandated for students in grades 7-11 and a second dose by 12th grade

REMINDER: Up-to-Date Immunizations Required to Attend School Parents/Guardians should make sure their children are up-to-date with their immunization/vaccinations before school begins in September. Please contact your pediatrician or clinic now for any necessary appointment. No child will be admitted to school without on time vaccinations.
Nonmedical exemptions to school vaccination requirements ended for children in Prekindergarten through 12th grade in New York State. This includes all public, private, and religious schools. Religious exemptions are no longer allowed.

If your child’s vaccinations are not up to date and complete, NOW is the time to contact your health provider or take your child to a local clinic.
The following health centers provide immunizations, prenatal, primary and other medical care for individuals who have no insurance. The sites also assist individuals in enrolling in eligible health insurance plans. Please call ahead for an appointment.

Immunization Forms

Proof of adequate immunization must include dates of vaccination on a stamped New York State physician certificate or be entered in the New York State database. Effective June 2019, New York State no longer accepts immunization exemption for religious reasons.
Children may be admitted to school provided they submit a physician-verified appointment to receive a mandated immunization within 2 weeks. School nurses will monitor compliance and students will be excluded from school if appointments are missed. Note: Students must be fully immunized before the first day of school.
