Parent/Guardian Account Request Form
The Yonkers Public Schools District can provide access to students information via the Home Access Center (HAC). In order to protect confidentially of students records, all parents/guardians who want to use this service are required to fill out this form and return it in person to any one of your child (ren)'s school(s). For security purposes, a photo ID is required when you return the form.
