School Health Clinic is Open for the Summer!
Cesar Chavez School, Eugenio Maria de Hostos Microsociety, Justice Sonia Sotomayor Community School, and Rosemarie Ann Siragusa Participation is voluntary and Registration is easy! Your child’s health is important, and now it’s easier to keep them healthy during the summer with our School-Based Health Programs. School-Based Health Clinic (SBHC) offer the same care as your local doctor’s office. Your child can receive medical and referral services, school physical and immunization at no cost to you. We work with your child’s pediatrician or primary care provider to keep your child in good physical and mental health to promote the best environment for learning. Enrolling your child and giving consent does not change your private doctor, your insurance or the number of times your child can see their primary doctor.
As the School Based Health Center we provide on-site services such as:
Comprehensive Physical Exam Sport physicals Immunizations and vaccines Screenings for vision hearing, dental and nutrition Mental health screenings Medical care for chronic conditions such as asthma, allergies, diabetes Routine sick visits and first aid
To register your child for the services at our School Based Health Center, please complete the attached Consent form, Basic Health History and Insurance form. Your child can return the completed forms to the school based clinic or the nurse’s office. We look forward to meeting you and caring for your child.
