After School Remote Learning Support: District Approved Medical Conditions

If your child has a medical condition that would compromise his/her health to attend in-person learning, the parent/guardian must do the following:

• Provide a medical note from the child’s primary care physician to the District’s Medical Office by emailing it to nrodriguez@yonkerspublicschools.org.
• Provide a phone number where the District’s Medical Office can contact you.
