Yonkers Public Schools is participating in the Community Eligibility Provision (CEP). We are in year 3 of the 3-year program. All children in YPS will receive breakfast and lunch at no charge to the student. This form is to determine eligibility for additional State and Federal program benefits for your child’s school.

Read the instructions on page 2, complete only one form for your household, sign your name and return it to Yonkers Public Schools Food Service Department – One Larkin Center, Yonkers, NY 10701 or your child’s school.
