
MEDICATION
If possible, all medication should be given at home.
In New York State, medicine (including over-the-counter medications) can be given in school only with a written physician’s order, which includes the patient’s name, medication name, dose, and time to be administered.
The parent or guardian must also provide a written request and deliver medicine to the school in a pharmacy labeled container. This procedure must be followed for prescription as well as over the counter medications.
The pharmacy labeled container/ medication must be brought to school in a bottle marked with:
- Name of student
- Name of physician
- Name of medication
- Time to be given
- · The dosage
If this procedure is not followed, medication will not be given.
