If your child has Medicaid coverage, your Medicaid plan will be sending a letter
regarding health insurance coverage.

WHAT CAN YOU DO TO MAKE SURE YOUR CHILD IS COVERED?

1. UPDATE YOUR CONTACT INFORMATION
NY State of Health
If you enrolled in Medicaid through NY State of Health, make sure NY State of Health has your current mailing address, phone number and email address so they can contact you about your health insurance. To update your information, call 1-855-355-5777.Westchester County Medicaid Office
If you enrolled in Medicaid through Westchester County’s Medicaid office, contact that office at 914-995-3333 to update your contact information.Has your contact information changed?
Contact the Westchester Social Services Department at 914-231-2100. -
2. CHECK YOUR MAIL
You will receive a letter via postal mail about your coverage. This letter will let you know if you need to complete a renewalform to determine if you still qualify for Medicaid or CHIP.

3. COMPLETE YOUR RENEWAL FORM (IF YOU GET ONE)
DO NOT DELAY!
Fill out the form and return it right away to help avoid a gap in your coverage.
If you lose Medicaid coverage, contact an enrollment assistor immediately to see if you can re-enroll in Medicaid or check to see if you qualify for subsidized coverage through the ACA Marketplace.

Make sure your address is up to date with NY State of Health or Westchester Department of Social Services so you receive your renewal letter in the mail.
Do you or your children have health insurance through NYS Medicaid or Children's Health Insurance Program (CHIP)? If so, here is what you need to know: be on the lookout for a renewal letter in the mail from NY State of Health or Westchester Department of Social Services.
To ensure you receive this critical piece of mail:
- Make sure your address, email and phone number are up to date with NY State of Health or Westchester Department of Social Services.
This Renewal Letter Will Tell You ONE of Three Things
- NY State of Health or Westchester Department of Social Services is RENEWING your Medicaid or CHIP coverage, OR
- NY State of Health or Westchester Department of Social Services is ENDING your Medicaid or CHIP coverage, OR
- NY State of Health or Westchester Department of Social Services NEEDS MORE INFORMATION to determine if you or your family member still qualify for Medicaid or CHIP. If so, a renewal form will be included.
